SW Community Health Center is a safety-net clinic providing basic health care to low- income uninsured individuals with a focus on SW Portland and the surrounding area.

6 Years of Service

In May 2011, SWCHC celebrated its 6th anniversary. Starting out as a clinic, located in the Multnomah Center and only open 2 nights a week, SWCHC moved to its present location – 7754 SW Capitol Highway in 2009. We currently have clinics Monday, Tuesday and Thursday as well as the first Wednesday of each month. We are approaching our 10,000th patient visit - providing acute care, preventive health services and chronic disease management to the community.

We believe a healthy community is a strong community – and put that belief into practice with the provision of quality, culturally-competent care.

Join us in supporting access to quality care. A donation of:

  • $144 – pays for a patient visit
  • $90 – pays for one month of diabetic supplies
  • $35 – pays for a complete lab panel
