Services

SW Community Health Center is a safety-net clinic providing health services to low-income uninsured individuals and families. We provide care in a safe, reassuring and culturally-sensitive environment.

We offer:

  • basic health services for treatment of acute health problems (i.e., infections, gastric distress, minor injuries)

  • ongoing care for most uncomplicated chronic diseases (i.e., diabetes, hypertension and asthma)

  • women’s health (including gynecologic exams and initial prenatal visit)

  • pediatric care (including well child check-ups and childhood immunizations)

  • patient education

  • preventive care

  • routine laboratory services for clients

  • management of mild depression

  • prescription medication (including pharmacy assistance) for clients

  • patient referrals for necessary care that exceeds our scope of practice

  • interpretation at the appointment (If you need an interpreter, please tell us when you call to make an appointment)

We are unable to offer:

  • treatment for acute asthma, shortness of breath or chest pain

  • treatment for acute pain with narcotics

  • management of chronic pain

  • prescriptions for narcotics or controlled substances

  • drug or alcohol counseling/treatment

  • management of complicated medical conditions

  • management of complicated mental health conditions

  • HIV testing or treatment

  • anti-coagulation management

  • ongoing prenatal care

  • care of medical emergencies

  • surgical, laceration repair, treatment of fractures and excisional biopsies

  • dental care

  • physicals for college, training programs, DOT, Peace Corps, motor vehicle claims, workman’s comp, disability, life insurance or sports physicals

Hours

  • SW Community Health Center is open Tuesdays and Thursdays from 4 PM - 8 PM and the 2nd and 4th Mondays of the month from 6 PM - 8 PM

  • We have a sliding fee scale - fees are based upon monthly (pretax income) and family size
