Services
SW Community Health Center is a safety-net clinic providing health services to low-income uninsured individuals and families. We provide care in a safe, reassuring and culturally-sensitive environment.
We offer:

basic health services for treatment of acute health problems (i.e., infections, gastric distress, minor injuries);

care managing asthma, diabetes and hypertension;

women’s health exams;
-
well-child check-ups;

preventive health services;
-
referrals for necessary care that goes beyond our scope of services;

interpretation at your appointment; (If you need an interpreter, please tell us when you call to make an appointment. We will arrange for an interpreter.)

limited lab services;
-
assistance with eligibility for health or pharmacy assistance programs.
Hours
SW Community Health Center is open Tuesdays and Thursdays from 4 PM to 8 PM and Mondays from 6-8 PM.
Appointments are required. Please call us at 503-977-0733. Your privacy is respected in accordance with health care standards.
A $15 donation is requested. This enables us to keep the clinic open so we can serve the community.
