Services

SW Community Health Center is a safety-net clinic providing health services to low-income uninsured individuals and families. We provide care in a safe, reassuring and culturally-sensitive environment.

We offer:

• basic health services for treatment of acute health problems (i.e., infections, gastric distress, minor injuries);

• care managing asthma, diabetes and hypertension;
  

• women’s health exams;
  

• well-child check-ups;
  

• preventive health services;
  

• referrals for necessary care that goes beyond our scope of services;
  

• interpretation at your appointment; (If you need an interpreter, please tell us when you call to make an appointment. We will arrange for an interpreter.)
  

• limited lab services;
  

• assistance with eligibility for health or pharmacy assistance programs.

Hours

SW Community Health Center is open Tuesdays and Thursdays from 4 PM to 8 PM and Mondays from 6-8 PM.

Your privacy is respected in accordance with health care standards.

A $15 donation is requested. This enables us to keep the clinic open so we can serve the community.
